- The New York City Community Health Survey (CHS) is a telephone survey conducted annually by the DOHMH, Division of Epidemiology, Bureau of Epidemiology Services. CHS provides robust data on the health of New Yorkers, including neighborhood, borough, and citywide estimates on a broad range of chronic diseases and behavioral risk factors.

- CHS data is available for each year on their website. Instructions on how to use the dataset is included on the webpage. Accompanying each dataset (downloadable in SAS) is the questionnaire, codebook and call in program and sample code for analyzing the data (using SAS and SUDAAN). Additionally, CHS includes the format program and the format statement program to assist in the downloading of the data.
