- The Racial and Ethnic Approaches to Community Health across the U.S. (REACH U.S.) is a CDC effort to eliminate ethnic and racial health disparities. The REACH U.S. Risk Factor Survey was conducted between 2009 and 2013 in order to monitor progress and achievements in the REACH U.S. program. The survey was conducted annually between 2009 and 2013 in 28 REACH U.S. grantee communities to gather health-related information in areas where REACH U.S. community health interventions were taking place. Adults aged 18 years or older in the REACH community were surveyed by phone, mail, or in-person. Communities surveyed included areas of Arizona, California, Georgia, Hawaii, Illinois, Massachusetts, New Mexico, New York, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Virginia, Washington, and West Virginia. Approximately 1,000 surveys were conducted in each of the 28 communities in each of the four years. Surveillance Summaries are available.
